GUIDELINES FOR SUBMITTAL OF ARIZONA TRAFFIC TICKET AND COMPLAINT FORMS FOR COURTS OR LAW ENFORCEMENT AGENCIES The Arizona Traffic Ticket and Complaint (ATTC) form is a multi-agency form that affects the public, the courts, and law enforcement. Therefore, law enforcement agencies are strongly encouraged to work with their applicable court(s) prior to submitting changes to ATTC forms. Rule 4, Rules of Court Procedure for Civil Traffic, Boating, Marijuana, and Parking and Standing Violations, requires each law enforcement agency or public body responsible for issuing ATTCs to submit any “substantial variations” to the ATTC form to the Supreme Court for approval. The substantial variation examples illustrate what is considered a substantial variation to an Arizona Traffic Ticket and Complaint (ATTC) form. This is not all-inclusive and may not address each agency’s particular situation.
