Order & Acknowledgment Forms

AMENDED, EFFECTIVE OCTOBER 14, 2013

Technical changes have been adopted to Forms 1 through 4 and Form 10.  These changes are effective October 14, 2013.  The technical changes include corrections to statutory references, consistent use of terms and formatting changes to assist in the ease of use and understandability of the forms.   No substantive changes have been made to the forms.  

By accepting appointment as a personal representative, guardian, or conservator, you have subjected yourself to the power and supervision of the court. The court will enter an order directing the performance of your duties in your fiduciary role. You are required to be guided by the order and comply with its provisions.

The appointing orders include:

 Form 1 – Order to Personal Representative and Acknowledgment and Information to Heirs/Devisees
 
 Form 2 – Order to Guardian and Acknowledgment and Information to Interested Persons

 Form 3 – Order to Conservator and Acknowledgment and Information to Interested Persons

 Form 3M – Order to Conservator of Minor and Acknowledgment and Information to Interested Persons

 Form 4 – Order To Guardian/Conservator and Acknowledgment and Information to Interested Persons
